Reflexiones sobre el impacto de la pandemia de la Covid-19 en el ejercicio médico y sus consideraciones bioéticas y legales / Reflections on the impact of the Covid-19 pandemic on medical practice and its bioethical and legal considerations

El presente trabajo tiene el objetivo de reflexionar, desde una plataforma bioética y legal, sobre las consideraciones que el médico en ejercicio debe tener presente ante el impacto de una emergencia sanitaria, como la pandemia de la covid-19, que le permitan responder con rapidez pero brindando una atención médica con previsión y calidad científica, capaz de contener la transmisibilidad del virus SARS-CoV-2 y así evitar que la capacidad hospitalaria sea rebasada, pero sobre todo para abordar adecuadamente los múltiples dilemas bioéticos que surjan y velar en todo momento por la dignidad, la integridad y la autonomía de las personas, con respeto a sus derechos humanos y en cumplimiento con la normativa vigente nacional e internacional. Estas consideraciones bioéticas y legales, si bien pueden no ser las únicas, son resultado del análisis discursivo, conceptual y crítico realizado, tres de los métodos empleados para el desarrollo de este trabajo.

The present work aims to reflect, from a bioethical and legal platform, on the considerations that the practicing physician must bear in mind in the face of the impact of a health emergency, which has been the case with the COVID-19 pandemic as of today. These must allow them to respond quickly, but they must also be able to provide medical care with foresight and scientific quality capable of containing the transmissibility of the SARS-CoV-2 virus and thus avoiding that hospital capacity is exceeded. Above all, the practicing physician must adequately address the multiple bioethical dilemmas that arise, and they must, always, ensure the dignity, integrity and autonomy of people, with respect for their human rights and, as well as comply with current international and national. regulations. These bioethical and legal considerations, although they may not be the only ones of their kind, are the result of the discursive, conceptual and critical analysis carried out, three of the methods used for the development of this paper.

Hipercalemia y su Abordaje Homeopático. Un Caso Clínico / Hyperkalemia and its Approach Homeopathic. A Clinical Case

La hipercalemia representa la complicación electrolítica más frecuente en la práctica médica. Este hecho adquiere gran importancia debido a su asociación a enfermedades como insuficiencia cardiaca (IC), diabetes mellitus (DM) o insuficiencia renal crónica (IRC), verdaderos retos para la medicina actual. Por ello, el caso clínico que se presenta resulta relevante, debido a que se trata de una paciente femenina de 72 años con IRC secundaria a una DM tipo 2 de 14 años de evolución que presenta descontrol en su glucemia, hipercalemia, anemia y gastropatía diabética manifestada por evacuaciones acuosas, indoloras y debilitantes en más de 10 ocasiones, por lo que fue hospitalizada. Su tratamiento intrahospitalario fue supervisado por el servicio de gastroenterología, nefrología, cardiología, medicina interna y nutriología. A su egreso, acude a consulta homeopática y presenta glucosa en límites normales (90 mg/dl). Sin embargo, continúa con hipercalemia leve (5.6 mEq/L) y síntomas propios de la gastropatía diabética que padece. Se le indica Kali carbonicum en potencia baja. Los resultados son positivos de manera inmediata y altamente promisorios en virtud de que baja el potasio a (4.6 mE/L) y mejora notablemente el cuadro clínico. (AU)

Therefore, the clinical case presented is relevant because it is a 72-year-old female patient with CRF secondary to a type 2 DM of 14 years of evolution, who has a lack of control in blood glucose, hyperkalemia, anemia and diabetic gastropathy. manifested by watery, painless, debilitating evacuations, more than 10 times, for which she was hospitalized. His in-hospital treatment was supervised by the gastroenterology, nephrology, cardiology, internal medicine and nutriology departments. Upon discharge, he goes to homeopathic consultation and presents glucose in normal limits (90 mg/dl). However, it continues with mild hyperkalemia (5.6 mEq/L) and symptoms of diabetic gastropathy. Kali carbonicum is indicated at low power. The results are immediately positive and highly promising because it lowers potassium to (4.6 mEq/L) and dramatically improves the clinical picture. (AU)

Efecto de la Homeopatía en un Paciente con Trombocitopenia Inmunitaria Primaria / Effect of Homeopathy on a Patient with Primary Immune Thrombocytopenia

Se presenta el caso clínico de un paciente masculino de 70 años, que acude en octubre del 2013 a la Unidad Médica Homeopática de la Escuela Nacional de Medicina y Homeopatía, del Instituto Politécnico Nacional (IPN), con el diagnóstico de trombocitopenia inmunitaria primaria, diagnosticada en diciembre del 2010 por el servicio de Hematología de un hospital de alta especialidad de la zona norte de la Ciudad de México, tratado con prednisona y danazol sin resultado, recomendando esplenectomía para la resolución de su problema de salud. El paciente es tratado con Arnica y Phosphorus en potencias bajas y medias respectivamente, medidas generales higiénico-dietéticas y retiro gradual y total de los medicamentos convencionales. Después de 22 meses de tratamiento homeopático las plaquetas se han incrementado a 112,000 por microlitro y no se han presentado zonas equimóticas, ni sangrados activos a otros niveles durante el tratamiento homeopático. El presente trabajo tiene el objetivo de mostrar el efecto terapéutico de algunos medicamentos homeopáticos en la rinofaringitis aguda o resfriado común, enfermedad aguda de vías respiratorias superiores de tipo viral de baja patogenicidad (VRSBP) y gran incidencia en la actualidad, en una muestra piloto de tipo clínico, longitudinal y prospectiva, conformada por 22 pacientes captados en la Unidad Médica de la Escuela Nacional de Medicina y Homeopatía (ENMH) del Instituto Politécnico Nacional (IPN); 15 pacientes del sexo femenino y 7 del masculino, en un rango de entre 4 y 11 años de edad. La elección de los medicament s homeopáticos se realizó con base en lasemiología de los síntomas presentados, enfatizando particularmente en las modalidades, el color del moco, ataque al estado general y la exploración física.

The choice of homeopathic medicament s was made based on thesemiotics of the symptoms presented , emphasizing particularly on the modalities , the color of mucus, malaise and physical examination.
